Motif CGM Patches are created with the patient in mind. We know how important it is for your device to stay in one place once placed on your skin. That's why we created the Motif CGM Patches. Simply peel, place and stick over your CGM device! For best results, we recommend changing your patch out approximately every 14 days. Patches are available for: Abbott Freestyle Libre 2, Abbott Freestyle Libre 3, Dexcom G6, and Dexcom G7.

Instructions:

• Clean your skin
• Peel off the paper tabs on the back of the patch
• Place the adhesive onto skin over the device and press firmly
• For best results, wait one hour after application before physical activity
• Each patch is designed to be used for up to 14 days with proper care.
• Without proper care patches may only last 5-7 days
• Please change your patch at the sign of wear, loss of adhesion.
• Warning: Do not apply to damaged or irritated skin. Do not reuse or re-wear used patches.
